Why can’t we just simply expand on the health care plans we already have rather than creating a whole new one?

We already have medicare and medicaid, why can’t we simply clean these up and expand who is covered under these rather than making a whole new health care plot? Medicaid already is pro-rated based upon income, why do we need a whole new health care plot?
